Commonly requested forms are listed below for your convenience.  If you are unable to access any of the documents, please contact the Registrar's Office.

Please note that third parties may not complete these forms.  Please see “Access to Student Records and Privacy of Information Policy” in the Academic Calendar for more information.  All forms and applications must be either presented in person, faxed, emailed or mailed to the Registrar’s Office.

  • Change Faculty Advisor - At any time you can change your faculty advisor.  This form must be filled out, signed and returned to the Registrar's Office.
  • Course Outline / Syllabi Request - Most course syllabi are saved and available to past and current students.  Please fill out the form and return it to the Registrar's Office.
  • Independent Study Request- Independent study courses are provided in most departments to allow qualified students the opportunity to explore a unique are of study not normally offered in the curriculum.
  • Letter of Permission Request - Full-time and part-time students registered at Redeemer may take courses at other institutions for credit in their degree program, if such courses are important for their program but are not offered by the university.
  • Name Change - Any requests to change a name, by means of alteration or deletion, substitution or addition, must be accompanied by appropriate supporting documentation.
  • Off Campus Studies Application
  • Proof of Enrolment Request - Fill out this request form in order to receive your RESP's, student loan, or for another purpose.  The letter takes approximately 3-5 days to process.  Please note that during September the processing time is long due to the volume.
