Commonly requested forms are listed below for your convenience.  If you are unable to access any of the documents, please contact the Registrar's Office.

Please note that third parties may not complete these forms.  Please see “Access to Student Records and Privacy of Information Policy” in the Academic Calendar for more information.  All forms and applications must be either presented in person, faxed, emailed or mailed to the Registrar’s Office.

• Blank Timetable  - Organize your schedule using this grid.
  

• Change Faculty Advisor - At any time you can change your faculty advisor.  This form must be filled out, signed and returned to the Registrar's Office.
  

• Course Outline / Syllabi Request - Most course syllabi are saved and available to past and current students.  Please fill out the form and return it to the Registrar's Office.
  

• Part Time & Occasional Studies Application - If you are planning on taking one, two or three courses at Redeemer, please fill out this application and return it to the Registrar's Office.
  

• Proof of Enrolment Request - Fill out this request form in order to receive your RESP's, student loan, or for another purpose.  The letter takes approximately 3-5 days to process.  Please note that during September the processing time is long due to the volume.
  

• Replacing your Degree Form - If you have misplaced your original degree and require a replacement.
  

• Request for an Incomplete Form - Only under exceptional circumstances will a student be granted an "incomplete" for a course.
  

• Withdrawl Notification Form - A student who wishes to withdraw from the university must completed this form and speak with Student Life.
